Enhancing Community Falls PreventionClasses under Your Health Notts is an initiative aimed at delivering additionalfalls prevention classes to areas of South Nottinghamshire. The programmefocuses on improving individuals' strength and balance to increase theirconfidence, reduce their fear of falling and promote ongoing maintenance ofphysical activity to foster sustainable behaviour change and overall positivewell-being beyond the end of the programme. Each session will also include timefor social connection and education, such as nutrition education, mental healthsupport and other ideas to support well-being related to the building blocks ofhealth.
Main duties of the job
Community Falls Classes Co-ordinator,you will lead the coordination and delivery of high-quality health and wellnessprogrammes, specifically focusing on the FaME and OTAGO programmes. Your rolewill involve engaging with the community to promote active and healthylifestyles, managing programme logistics, capturing and reporting data, andcollaborating with stakeholders to ensure the success and sustainability of theprogrammes.

Main Duties and responsibilities:
Programme Delivery and Management:
Coordinate and oversee the delivery ofthe FaME programme in Eastwood (24-week programme).
Support instructors in setting up anddelivering the FaME programme.
Delivering the existing OTAGO class inHucknall.
Set up and deliver new OTAGO classesin Arnold and Cotgrave, ensuring each runs effectively for 12 months.
Engage with community groups to ensureeffective promotion/signposting/referrals to and from offers they alreadyprovide seated exercise; Tai-Chi; walking groups, etc.
Data Management and Reporting:
Capture accurate data from outcomemeasures and goal-setting targets in line with the commissionersspecifications.
Monitor participant progress andprovide ongoing support throughout the programme.
Prepare detailed reports and presentdata during performance review boards and contract reviews.
Support the ongoing evaluation of thisinitiative as required.
Client Support:
Provide one-on-one and group supportto programme participants.
Utilise motivational interviewing andpositive psychology techniques to encourage sustainable behaviour change.
Monitor and track client progress,offering personalised advice and adjustments to their wellness plans.
Collaboration and Networking:
Establish and maintain strongrelationships with community stakeholders.
Work collaboratively with themulti-disciplinary team to enhance service delivery and client outcomes.
Liaise with GPs and other relevanthealthcare professionals to ensure comprehensive client care.
Establish strong relationships withrelevant local authority colleagues Health and Wellbeing officers, Local LeisureCentre providers, and Adult Social Care teams.
Administrative Duties:
Maintain accurate records and data inaccordance with organisational policies and procedures.
Prepare and contribute to reports onprogramme outcomes and impact.
Ensure all activities andinterventions comply with relevant guidelines and standards.
Continuous Improvement:
Stay up to date with the latestresearch and best practices in health promotion and community wellness,specifically falls prevention.
Contribute to the development of newresources and materials to support programme delivery.
Participatein training and professional development opportunities to enhance skills andknowledge.

Qualifications
Essential
1. Educated to degree level OR demonstrable relevant equivalent experience.
2. Qualified to level 3 personal training or equivalent.
Desirable
3. Degree in nutrition/exercise, psychology/public health or other health-related field.
4. Sports Coaching qualification.
Skills and Knowledge

5. Excellent written, verbal, and non-verbal communication skills, with the ability to build rapport quickly with people.
6. Excellent administration, record keeping, and data collection skills.
7. To have an understanding of the wider determinants of health Building Block of health
