The role will include working across community venues, in patients' homes, being a core member of the multi-disciplinary team and empowering patients to make informed choices about their health and health care. This role demands the ability to be innovative, flexible and a strong team player and be able to work autonomously. Due to the nature of the role, it is essential that applicants can travel independently across the locality and have access to transport for work purposes. You will need to work within your own scope of practice and be able provide pulmonary and exercise rehabilitation support to patients with a Respiratory diagnosis.
