Your role and responsibilities You will: lead delivery of the Healthy Child Programme for ages 0-5 conduct assessments and care planning evaluate outcomes and support service improvement champion health promotion and reduce inequalities provide health reviews and targeted support to families maintain NMC registration and work to The Code safeguard children through collaboration, assessments, support and advice be the key contact and case holder for families.
