Unable to show this video due to your cookie preferences What You'll Be Doing As a Social Worker in our Adult Social Care teams, your key responsibilities will include: Strengths-Based Assessments : Conducting assessments to understand the strengths and needs of individuals, helping them achieve their personal goals and promoting their independence. Support Planning : Developing and implementing support plans tailored to each individual's needs, ensuring they receive the appropriate services and resources. Reviews : Regularly reviewing support plans to ensure they remain effective and making adjustments as necessary to meet changing needs. Collaboration : Working closely with a multidisciplinary team, including healthcare professionals, community organizations, and other local authority teams, to provide comprehensive support. Promoting Independence : Encouraging and enabling individuals to be as independent as possible, supporting them in making their own decisions and managing their own lives. Legal Compliance : Ensuring all practices comply with relevant legislation, such as the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 2007, Human Rights Act 1998, and Equality Act 2010. Continuous Improvement : Engaging in continuous professional development to stay updated with best practices and new developments in social work.
