Job summary An exciting opportunity has arisen for an experienced Practice Nurse to join our friendly, supportive nursing team. We are looking for a motivated, committed team member with a can-do-attitude to support our caring nursing team in the delivery of patient centered health care, which will include a range of long-term conditions, immunisation and vaccination, health promotion and screening. Washway Road Medical Centre is a dedicated training practice and would be willing to support the correct candidate in increasing their skills and knowledge base. Main duties of the job The main duties of the role will include but not limited to: Be competent in phlebotomy, health checks and routine monitoring Undertake NHS immunisation programmes for adults and children Support chronic disease management including Asthma, COPD and Diabetes Maintain practice stock and supplies Chaperone and assist patient where appropriate Participate in practice QOF, quality contract and DES requirements About us Washway Road is a large, busy yet caring General Practice. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels: a. To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing, training, support and evidence of compliance to the GP IPC lead, Lead Nurse for IPC and the Practice Manager b. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing c. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice d. To implement vaccination programmes for adults and children and support with patient audit and recall. e. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health f. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF g. To prescribe medication for patients in accordance with PGDs and own scope of practice h. To review patient medications to enhance compliance i. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages as a part of chronic routine care within scope of practice j. To request pathology services and provide phlebotomy services k. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required l. To maintain accurate clinical records in conjunction with extant legislation m. To ensure snomed codes are used effectively by all members of the nursing team / accurate clinical coding to support QoF n. To ensure the appropriate maintenance and use of chronic disease registers o. Support the practice with ordering of clinical supplies, immunisations and equipment. Ensuring effective stock control and ensuring supportive documentation. p. to ensure compliance with the practice chaperone policy q. To assist GPs with minor surgery/coil clinics when required r. To prioritise health issues and intervene appropriately s. To support the team in dealing with clinical emergencies t. To recognise, assess and refer patients presenting with mental health needs u. To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice) v. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.) w.
