As part of our services across Oxfordshire, we are implementing Primary Care Mental Health hubs to support GP practices with Mental Health assessment and support for patients in their communities.
We have created new and exciting posts within Keystone Mental Health and Wellbeing Hubs. This role will be based within the Keystone Mental Health and Wellbeing Hub in Banbury.
Main duties of the job
We are looking for Integrated Mental Health Hub Clinicians who are experienced in delivering evidence-based interventions in a community setting, with a passion for partnership approaches to care for adults living with serious mental illness.
These roles need people who are innovative and want to help achieve our vision for cross-sector integrated pathways, removing existing barriers, to deliver better access to dedicated mental health support in local, non-stigmatised community settings.

Detailed job description and main responsibilities
12. Supporting the Clinical Lead in the development and implementation of the service and your role within it.
13. Supervision and leadership for appropriate staff identified
14. Screen, triage, assess and coordinate referrals
15. Provide brief support and time limited evidence-based interventions,
16. Support patients ready to transition to primary care and develop care plans and prepare people for step down.
17. Map community resources and assets that are available for those with SMI within the specific PCN geography.
18. To support collaborative working between the full range of physical health and mental health services
Person specification
Qualifications
Essential criteria
19. Current professional registration: Registered Mental Health Nurse, Occupational Therapist.
Desirable criteria
20. Post graduate education specifically relating to client group.
Experience
Essential criteria
21. Community experience in mental health services for adults and older age adults with severe / significant mental illness.
Desirable criteria
22. Experience of working in primary and secondary mental health services.
Knowledge
Essential criteria
23. Specialist knowledge of person-centred care and its application.
24. Specialist knowledge of psychosocial interventions and their appropriate use.
25. Understanding of the mental health needs of adults.
Desirable criteria
26. Specialist knowledge relating to MH needs of young adults age 18-25 and to the needs of those diagnosed with, or with presenting traits of, personality disorder.
27. All new starters have a thorough induction process, both local and Trust wide, which aims to create a positive supportive working environment allowing new employees to learn key elements of their role over a reasonable timescale.
