We are looking for an enthusiastic and adaptable Children’s Community Nurse to join our caring and progressive team in Faversham.
You will work as part of a close-knit team who are dedicated to the health and wellbeing of people in Faversham. Faversham is made up of a diverse patient group with complex needs.
District nurses make a difference by making sure our services are proactive and patient-focused, helping to keep people at home and independent for longer.
Our community nursing teams are closely aligned with local Primary Care Networks, to make sure patient care is holistic and joined up. We believe in treating the whole person, not just the condition, so we take time to get to know our patients.
Main duties of the job
Community nurses provide complex nursing care to people at home.
As a community nurse, you'll
1. assess the healthcare needs of patients and families
2. monitor the quality of care they're receiving
3. be professionally accountable for its delivery
As a Children’s Community Nurse you will work as part of a team to keep people safe and well at home. You will use your clinical judgment to support evidence-based care planning for people living at home, in care homes or other settings and make sure that people who use our services have a good experience.

Qualifications
Essential criteria
4. Registered Nurse (Child Branch) or RSCN. Evidence of relevant post-registration training
Desirable criteria

Experience
Essential criteria
6. At least 2 - 3 years post-registration clinical experience working with children and their families. (Alternatively 12-18months post-registration clinical experience and approx. 12 months working as a Band 5 Associate Community Children’s Nurse) Wide range of professional knowledge and experience of paediatric nursing including palliative care, oncology, respiratory, cardiac, gastroenterology, special needs, complex care and experience of working with children/young people with additional needs, including those with life threatening/life limiting conditions and long term conditions in a variety of settings and an awareness of the child/young person’s physical, emotional, psychological and social needs and how best to meet them.

7. Experience of palliative care within the community setting

8. Sound nursing knowledge and experience of all basic clinical nursing skills, including care of Tracheostomies, enteral feeding devices & Central venous catheters.
