An exciting opportunity has arisen for a hardworking, caring and compassionate individual to work as part of the Oncology Clinical Nurse Specialist Team for Swansea Bay University Health Board.
The band 4 coordinator for prostate cancer will provide a vital role in supporting the CNS with the management of this patient group.
The South West Wales Cancer Centre (SWWCC) delivers a service to all patients across South West Wales and covers two large health boards, Swansea Bay and Hywel Dda.
The post holder will work under the guidance and support of the prostate cancer clinical nurse specialist to provide a holistic, sensitive and compassionate service to support and meet the needs of our patients.
This role will support the patient along the diagnostic and treatment pathway and after treatment has been completed.
It will involve supporting aspects of care delivery, patient pathway co-ordination, assessing the needs of patients and carers and providing information, help and support to those living with prostate cancer.
As an extended member of the prostate MDT, the role will support effective communication between patients, their families and carers, primary and secondary care and with third sector agencies., The Band 4 coordinator will be expected to support the clinical nurse specialist in managing the needs of patients diagnosed with prostate cancer.
The ideal candidate must demonstrate excellent communication skills, be motivated and able to work in a team and use own initiative., The post holders require a willingness to be flexible in order to meet the needs of the service.
NVQ level 3 is essential with the willingness to work towards level 4.
The ideal candidate will have had previous experience of working within health care, to support with the assessment, planning, implementation and evaluation stages of the patient's treatment plans.
You must have excellent communication/interpersonal skills and the ability to work on your own initiative as well as part of a team.
