Position: Advanced Clinical Practitioner\nSalary: up to £65,000 + unsociable hours\nEmployment type: Permanent, Full / Part Time available\nLocation: Leeds\n\nJob Description:\nThe Advanced Nurse/Clinical Practitioner (ANP/ACP) utilises expert clinical and\nknowledge to manage patients presenting with acute, undifferentiated\nproblems in addition to those with chronic long term conditions.\nWorking within their defined scope of practice, the ANP/ACP works as an\nautonomous Practitioner to assess, diagnose and plan care for patients of all\nages, including infants. The post holder would assess and treat minor injuries\nand minor illness and will prescribe medications where qualified to do so, and\nwhere clinically indicated.\nWorking across Urgent Care Services, the ANP/ACP will manage acute\npresentations to the Walk-In Centre, as part of a team of autonomous\nPractitioners; in addition to using the clinical knowledge and skills to\ndetermine urgency and best pathway for patients presenting to the Emergency\nDepartments (streaming).\n\nMain Responsibilities:\n• To assess patients at point of entry into the health care system with\nundifferentiated undiagnosed problems.\n• To provide care within their skills set across the full range (infants, children,\nadults and elderly)\n• To assess and treat minor injuries and minor illness\n• To take responsibility for making autonomous decisions in relation to patient\ncare.\n• Using advanced assessment skills of history taking and clinical examination,\nmake a diagnosis and initiate an appropriate care plan including any necessary\ndiagnostic tests of referral\n• To promote health and screening in relation to primary prevention. Gain an\nunderstanding of chronic disease management and access training as\nappropriate\n• To prescribe within current parameters according to prescribing authority for\nextended and supplementary prescribing.\n• To participate in the development and implementation of service/CCG\nguidelines and protocols.\n• Provide expert nursing clinical advice to colleagues Including mentoring of\njunior practitioners.\n• Work with nursing and medical teams in providing a proactive management of\npatients with chronic disease according to local strategies.\n• Attend training events to maintain professional development.\n• Ensure confidentiality is maintained at all times as per Caldicott requirements \n\nAdvanced Physical Assessment and Consultation Skills\nLevel 7\n\nEssential\nMinor Illness – Level 7 Essential\nNon-Medical Prescribing – Level 7 Essential\nMinimum 3 years’ experience post level 7 qualifications Essential\n\nDesirable\nPaediatric Course – Level 7 Desirable
