About the Role
As a Community Reablement Worker in the Learning Difficulties & Autism Team, you will deliver a community-based enablement and inclusion service, which supports people to learn or relearn skills, reduces social isolation, and encourages young people to become part of their community and as independent as possible. You will work with individuals as they transition to adult services and the key people in their lives to identify specific goals and outcomes which you will then translate into a progression plan, setting out how these will be achieved and including strategies that can be used to manage any identified risks.
As part of your role, you will support individuals to explore a range of community options such as access to employment, voluntary opportunities, recreational/social pursuits, and community, cultural or spiritual activities. Where necessary, you will provide information to commissioning workers about an individual's eligible needs as well as identifying and reporting potential safeguarding, mental capacity, and other high-risk situations where a person's safety or liberty may be at risk.
This is a part-time, 18.5 hours per week post. A DBS enhanced check for a regulated activity is required for this post.
