Job overview
We have an exciting opportunity for a Registered Nurse who has the skills required for the role of a frailty nurse practitioner. You will proactively assess/case manage high risk frail patients with chronic long term conditions and complex health and social needs to avoid unnecessary hospital admission.
The post holder will provide advanced clinical assessment, needs identification, planning, case and medicine management and co-ordination of care for people with long term conditions who have highly complex needs and co-morbidity.
The post holder will work towards advanced professional practice including leadership, improving mental health and supporting self-care management. The post-holder will promote partnership working to improve outcomes for patients with a co-morbidity of long term conditions in order to reduce unplanned hospital admissions, reduce length of stay, and facilitate seamless transition of care.
The post holder will work flexibly managing patients across multiple pathways dependent on need.

Qualifications
* Degree in appropriate subject or evidence of education to degree level.
* Current NMC registration
* Clinical Examination and Clinical Diagnostic CPD Modules.
Knowledge
* High level of clinical knowledge including current evidence-based practice
* Knowledge of Chronic Disease Management
* Knowledge and understanding of the principles of governance and its implications to their service area.
* Knowledge and appreciation of lone working risks – policy and procedure.
Experience
* Experience of stakeholder negotiation and persuasion skills.
* Ability to communicate highly complex pathologies and diagnostic reasoning to all levels in the medical and Non-Medical field at the appropriate level.
* Sound knowledge of psycho-social problems and ability to gain patient confidence and co-operation.
