Maintaining patient records and producing care plans and risk assessments, including assessment of the physical health of patients. Working with patients' families and carers, helping to educate them and the patient about their mental health problems.
Liaising with patients, relatives and fellow professionals in the community treatment team. Identifying whether and when patients are at risk of harming themselves or others. Ensure good networks are established with partner organisations and signposting as appropriate.
Within the scope of your professional qualifications and experience, take part in in-house CBT and other psychological therapies or take steps to develop this. Provide support to other pathways such as opioid dependence, end of life, pain control, and chronic disease management.
