* Significant post-registration experience at Band 5 or equivalent, some of which should be within renal medicine.
* Recognised Teaching/Assessment qualification.
* Recognised independent prescriber qualification, or willingness to undertake.
* Excellent communication and interpersonal skills.
* Evidence of multidisciplinary team working.
* The ability to work both autonomously and as part of a team.
* A full UK Driving Licence.

Main duties of the job:
The post holder will be responsible, alongside two other established CNS, for patients with chronic kidney disease within community clinics located in GP practices across Leeds and Mid-Yorkshire. The post-holder will be part of a team responsible for anaemia management of the patients reviewed within these clinics. The post holder will demonstrate evidence-based care in the independent assessment, care planning, treatment and evaluation of a patient group presenting with stable chronic kidney disease, within agreed protocols. The nurse-led clinics are both face-to-face and telephone, and are facilitated by Consultant Nephrologists. In addition to the commitment to outpatient review of patients, the post holder will assist in the promotion of developments of chronic kidney care in both primary care and the community.
About us:
The Leeds Renal Services lies within the Abdominal Medicine Surgery (AMS) Clinical Service unit at Leeds Teaching Hospital Trust. The renal service receives referrals for adults for the management and treatment of disorders of the kidney. The spectrum of disease ranges from mild disorders to life-threatening conditions. All the renal consultants see patients with general nephrological problems and, in addition, have their own special interests. The main renal unit is at St James's University Hospital, where inpatient, outpatient, pre-dialysis, haemodialysis, peritoneal dialysis and transplantation services are provided. Nurse-led outpatient clinics also take place at St James's. The sub-regional kidney transplant service for West, East and North Yorkshire is located at St James's University Hospital.
