Proposed salary: Band 5.1 – 5.10 on the ABC pay scale, which is equivalent to £26,747.94 - £31,966.27 per annum dependent on experience (pro rata)
Hours of work: Part time (up to 30 hours per week)
Base: Holbrook Surgery and Riverside Medical Practice

About Horsham Central PCN
Horsham Central Primary Care Network is a NHS Collaboration between two GP Practices – Holbrook Surgery and Riverside Medical Practice - working together to provide enhanced access services. Our surgery teams work closely, sharing expertise and resources to develop new services.
Job summary
The post holder will work across the practices with each of the practice teams, joining the PCN team for Central Horsham. They will be assigned a lead clinical and operational contact for each contract.
This post is part of the Additional Roles Reimbursement Scheme for Primary Care Networks funded by NHS England and includes funded access to the Primary Care Pharmacy Education pathway pilot for pharmacy technicians (where appropriate).
Primary duties and areas of responsibility:
Clinical
* Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.
* Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation.
* Support the Clinical Pharmacist in Structure Medication Reviews (SMR).
* Provide expertise to address both the public health and social care needs of patients.
* Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients.
Technical and Administrative
* Support the PCN multi-disciplinary team to ensure efficient medicines optimisation processes.
* Implement efficient ordering and return processes and reducing medication wastage.
* Provide training and support on the legal, safe and secure handling of medicines.
General
The postholder will:

Minimum Requirements:
The post-holder must have a BTEC/NVQ level 3 or equivalent in pharmaceutical sciences and evidence of continued professional development (CPD).
