Local Primary Mental Health Practitioner
An exciting opportunity has arisen in Primary Mental Health Services for an experienced, motivated and forward-thinking practitioner to join a busy supportive team based in the West area.

This position has arisen with the South Community Mental Health teams at an exciting time where services are under review and development in order to meet the changing needs of both patients and carers.
The successful candidate, whilst working closely with a multi-disciplinary team, will receive referrals directly from GPs and provide short-term interventions for individuals with mild to moderate illness. They will work closely with colleagues from secondary care and specialist services as well as third sector organizations and statutory agencies.
The successful candidate will manage a caseload of patients with whom they will work alongside to formulate risk management plans and care planning based on a robust assessment of need.
We are looking for a good team player who has excellent communication skills and the ability to prioritize and manage their time effectively.
Main Duties of the Job
1. Provide initial mental health assessments in line with the agreed BCUHB assessment procedures for the LPMHSS.
2. Provide time-limited interventions in the primary care settings for patients with mild to moderate mental health problems.
3. Act as the first point of contact for GPs, offering advice, support, and signposting to appropriate agencies. Manage their own therapeutic care/interventions within the clinical environment and be accountable for the quality of care they deliver within the clinical area they are responsible for.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Person Specification
Qualifications and Knowledge
* NMC registered Nurse (Part 3 or Part 13), social care Wales or HCPC.
* Up-to-date working knowledge of the Mental Health Act (1983) and the Mental Health Wales Measure 2010.
* Knowledge of and application of Clinical Governance and evidence of continuing professional development.
* Up-to-date knowledge and application of consent, data protection, patient confidentiality, and risk management with health and safety issues.
* Knowledge of national strategies related to the sphere of nursing.
* Knowledge of Information technology and application.
* Evidence of further relevant professional development (Diploma or degree in relevant clinical/professional subject).
Experience
* Post-registration experience at Band 5 or above.
* Post-registration experience required at postgraduate level.
* Experience within relevant and varied clinical areas.
Attitude and Abilities
* Excellent communication skills.
* Enhanced assessment skills (e.g., ACEIII, GDS, Risk assessing).
* Counseling skills.
