Current Services Inpatient unit A 12 bed Inpatient Unit at John Eastwood Hospice, providing facilities for symptom management, assessment, and terminal care. The average length of stay is 12 days and 50% of patients are discharged to home or alternative place of care. A weekly MDT is held to discuss the in-patients. The proximity of the hospice to Kings Mill Hospital allows easy access to investigative procedures for hospice patients. Hospital Consultant colleagues visit the hospice to assess patients as required. Outpatients Outpatient clinics are held at John Eastwood Hospice and Newark Hospital. Hospital Support Team The hospital support team provide advice and support for patients in the Acute Hospital and Community Hospitals. The team consists of Clinical Nurse Specialists and Consultants. We hold a weekly MDT. We contribute to the training and development of palliative and end of life services at SFHT. Community SPC Outreach Service A Community MDT is based at John Eastwood Hospice and a further team at Newark Hospital. They work closely with primary care teams and hold weekly MDTs with Clinical Nurse Specialists, Consultant and training grade doctors, and other health professionals. Non-malignant palliative care Whilst most of the work remains cancer-related, there is an increasing commitment to non-malignant palliative care, including assessment and symptom management for patients with neurological conditions, heart failure and chronic lung disease.
