The Colman Centre Specialist Rehabilitation Service requires an experienced Assistant Practitioner as a Discharge Co-ordinator to support the day-to-day supervision and co-ordination of the patient's pathway from admission to discharge and provide a troubleshooting supportive role to the multi professional team. The Inpatient Service provides rehabilitation to support patients either to return home, move to an alternative safe environment or to continue their rehabilitation pathway in the Community. You will be required to develop strong links with other members of the multi-disciplinary team involved in the discharge process and the on-going care needs of individual patients in the Community. You will be expected to use your own initiative and be able to work unsupervised, undertaking specific work including audits as necessary, as this role is pivotal to driving this service forward. Main duties of the job To independently and actively monitor the multi-disciplinary management and discharge plan for all patients and take action to expedite the process, avoid delays and thereby ensure efficient timely discharge. Taking an overarching role in supporting patient flow through the system. A critical success factor in this role is reducing the length of stay for all patients.
