We have an exciting opportunity for a qualified Band 6 Social Worker to join our welcoming, dynamic multi-disciplinary Specialist Mental Health Service for Older Adults, serving the needs of older people within North Hertfordshire, covering Stevenage and surrounding villages.
You will be responsible for delivering care to an allocated caseload of service users experiencing or recovering from mental health difficulties. Responsibilities include completing holistic assessments, developing innovative support plans, reviews, and CPA in collaboration with other professionals, while building networks, raising awareness, and developing valuable links for the team.
You will work alongside Consultant Psychiatrists, Psychologists, Nurses, Occupational Therapists, and Support Workers to achieve the best outcomes. You will provide direct support and advice to service users, their families, and carers within the recovery framework, respecting individual rights to choose and control.

This role offers a Golden Hello of £3,500 for new employees, paid as £1,500 on commencement and £2,000 after one year of employment.

Main duties of the job
You will demonstrate knowledge of relevant legislation, such as The Care Act 2014, The Mental Capacity Act 2005, and The Mental Health Act 1983 (amended 2007).

You will champion social care, understanding legislation and statutory guidance, promote adult safeguarding, undertake safeguarding inquiries, and contribute to care plans that protect adults and children from abuse.
You will undertake comprehensive assessments and reviews, especially for high-risk individuals or those with complex social, legal, or environmental needs, ensuring appropriate assessment against social care eligibility criteria.
You will provide direct support and information to service users and carers, signposting those not eligible for social care to appropriate services.
You will ensure health and social care services are commissioned appropriately, including direct payments and charges where applicable.
You will conduct risk assessments to ensure the safety of vulnerable individuals and undertake carer assessments with contingency planning and support provision.

Person specification
KNOWLEDGE, TRAINING AND EXPERIENCE
* An appropriate professional social work qualification (e.g., DipSW), Education to degree level
* Registration with Social Work England
* Training in outcomes-focused assessments and personalization principles
* Relevant post-qualifying training for social workers
* Safeguarding training and experience
Desirable criteria
* Approved Mental Health Professional/Best Interests Assessment Training
AREAS OF EXPERIENCE AND KNOWLEDGE
* Minimum of 2 years post-qualification experience
* Knowledge of relevant legislation, theory, and policy
* Experience in assessments, care planning, and personalization
* Understanding of personal budgets and direct payments
* Experience working collaboratively in multidisciplinary teams
* Partnership working with service users and carers, demonstrating dignity and respect
Desirable criteria
* Experience as an Approved Mental Health Professional or Best Interests Assessor
