The Integrated Discharge Team is responsible for supporting patient discharges for the residents of Salford from Salford Royal Hospital and out of area hospitals. This team is also responsible for supporting patients discharged from the acute settings through the bed based Discharge to Assess program in support of patients returning to their community settings.

As a Community Assessment Officer at the forefront of service delivery, you will need to focus upon enabling and maximising independence for people living in Salford.
Working alongside a large Multi-Disciplinary Team, the role is hospital and community based: collaboratively completing strength-based assessments and reviews that are designed to support people to identify their own individual needs and desired outcomes in supporting them to improve their wellbeing. You will also commission packages of support that complement the person's existing natural support in the community. Support is aimed at building upon a person's own strengths and networks to meet need, build independence and ultimately enable them to achieve their goals and ambitions.
You will be fully supported with regular supervision and training opportunities. Real career progression for Community Assessment Officers is a priority in Salford. Once established, and after a period of one year in employment: Community Assessment Officers will have the opportunity to apply to the Social Work Apprenticeship Scheme that has been running for the past two years.
