Community Alcohol Support Worker (CHAT)

Job overview
The Community Hospital Alcohol Team is a community-based team working closely with UHNM to provide alcohol support to the community of Stoke on Trent for patients that attend emergency portals with harmful use of alcohol.
We reduce the risk associated with alcohol and promote change to improve the health and social welfare of patients. The community alcohol support worker (CHAT) will support the alcohol detox nurses within the team. This post is part-time, 30 hours per week.
We offer a 7-day nurse-led service and our hours are 8am - 4pm providing increased support as required.
We provide Psychosocial Interventions for up to 12 weeks from discharge at UHNM, this being harm reduction, relapse prevention, and seamless referrals on to recovery services.
Main duties of the job
1. Support earlier discharge from hospital / admission avoidance for suitable patients with alcohol-related problems.
2. Assist Service Users who are utilizing multiple emergency services i.e. ambulance, A&E, IOU, Harplands, crisis teams.
3. Provide increased support for service users with co-existing complex physical health needs (e.g. chronic liver disease, end of life) that require a coordinated case management approach.
4. Deliver increased support for people known to be difficult to engage in services and who require an assertive management approach (e.g. require home visits, greater number of contacts to remain engaged).
5. Assist Registered Professionals in assessments and care planning/implementation and evaluation of individualized care plans.

Detailed job description and main responsibilities
1. Communicate with service-users and carers, all members of the multi-disciplinary team, and staff in external agencies using a variety of methods including Information Technology.
2. Regularly report and effectively communicate service-users' health status and care aims in Halo System and providing verbal reports to Registered Health Professional staff members and other members of the multi-disciplinary team.
3. Relate appropriately with service-users, carers, colleagues, and others – at all times maintaining professional boundaries.
4. Ensure that own actions support the equality, diversity, rights, and responsibilities of individuals.
5. Identify, minimize, and report risks of danger, harm, and abuse upwards to senior colleagues.
6. Minimize risks to self and others by undertaking safe working practices.
7. Work within agreed and established Trust policy and operational procedures.
8. Collaborate with the Royal Stoke Emergency Department and other services to promote seamless service-user person-centered care from referral, transfer, and discharge through the identified care pathways.
9. Contribute towards planning and undertaking delivery of planned therapeutic activities with an identified group of service users.
10. Undertake elements of a specific alcohol and holistic assessment, under the direction of Registered Health Professional staff.
11. Work in partnership with service-users to assist them in identifying recovery planning that is meaningful to them and supports agreed health outcomes.
12. Support service-users to access a range of activities and services to maintain their independence and promote their health and wellbeing.
13. Provide advice and guidance to Service-users – promoting healthy activities and lifestyle choices.
14. Support Registered Practitioners and Service-users during activities to meet their physical substance misuse and mental wellbeing.
15. Assist service-users in meeting their assessed personal care needs.
16. Provide information, support, and guidance to Carers, Friends, and Relatives of service-users, maintaining appropriate confidentiality.
17. Take responsibility for own personal and professional development and support the development of colleagues, students, trainees, and volunteers.
18. Access and make use of supervision regularly.
19. Participate in activities focused on the delivery of a high-quality customer-focused service.
20. Undertake activities to improve services.
Person specification
Qualifications
* Advanced Apprenticeship in Mental Health and Social Care or NVQ 3 in substance misuse with demonstrable evidence of numeracy and literacy skills (equivalent GCSE's or Functional Skills).
Experience
* Experience of working with people with substance misuse needs as a member of a multi-disciplinary team.
