Allen Street Clinic is lookingfor an enthusiastic and motivated Practice Nurse to join our friendly team.
Applicants must be fullyregistered with the NMC and have excellent communication skills. Previousexperience working as a nurse in a GP Practice is desirable, however fulltraining and ongoing support will be provided for the right candidate.
The Practice Nurse will ensure that allnursing care provided is a high-quality service based on the needs of thepractice population which includes treatment, screening, preventative care,chronic disease management, vaccinations and immunisations, patient educationand advice.
Main duties of the job
The Practice Nurse will bean integral part of the practice team forming effective relationships withdoctors, staff and patients. You will berequired to undertake all general nursing duties in accordance with the NMC Code ofProfessional Conduct and practice protocols and establish programmes of care for patient groups withcontinual overall nursing requirements.

Ourclinical team consists of 2 GP Partners, a Salaried GP, and practice nurses. They are supported by an experienced anddedicated team of managerial, administration, reception and dispensary staff. We are further supported by our PCN Staffwhich includes care co-ordinators, pharmacists and pharmacy technicians, socialprescribers, mental health practitioners and health and wellbeing coaches

We alsoparticipate in Research and are a GP Training Practice, working incollaboration with Keele Medical School, having GP Registrars and MedicalStudents on placement with us throughout the year
Job responsibilities
MainDuties & Responsibilities
Workingwithin professional competencies and following practice policies, protocols andguidelines, duties will include:
* Supportand enhance the patient care offered by the GPs
* Provideroutine nursing care to patients including venepuncture, BP management,urinalysis, blood glucose testing, ECGs, cervical screening, wound management,taking swabs
* Undertakeall the normal duties and responsibilities associated with a practice nurseworking within primary care, including participating in all aspects of QOF, QIFand all enhanced services
* Advise,educate and develop ongoing healthcare with an emphasis on preventativemeasures
* Supportand educate patients on their condition and development of self-managementplans where appropriate
* Assessthe needs of patients, to ensure the provision of healthcare is appropriate andprovide safe, evidence based, cost effective, individualised patient care
* Identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to patients health
* Workautonomously in providing the care and management of chronic disease careprovision and maintain standards of care for patients with acute and chronicmedical conditions using national and local guidelines where appropriate
* Initiateand carry out programmes of health screening promoting health and wellbeing,giving general education and advice on weight, diet and healthy eating,activity and exercise, smoking cessation, alcohol consumption and chronicdisease management
* Providehome visits to patients who are housebound to complete relevant assessments andtreatments as necessary
* Administerchild and adult immunisations and vaccinations in accordance with the GMScontract and following national and local programmes
* Provideup to date contraceptive and sexual health advice
* Requestappropriate pathology tests and process results as required
* Provideassistance and chaperoning to GPs when required including in minor surgery andcontraception clinics and other procedures
* Providecomprehensive travel health advice and vaccinations for patients prior totravel
* Supportcolleagues in dealing with clinical emergencies
* Maintaindisease registers in liaison with administrative staff
* Actas Infection Control Lead and support clinical colleagues with InfectionControl including reviewing policies, producing audits, training and evidenceof compliance

Otherresponsibilities within the practice:
* Towork flexibly within the team and to provide cover as necessary
* Prepareand complete the revalidation process and ake responsibility for ownprofessional development, learning and performance and demonstrate skills andactivities to others who are undertaking similar work
* Participationin an annual individual performance review and appraisal within the practice
* Attendtraining and events organised by the practice or other agencies, whereappropriate, and ensure all mandatory training is completed
* Complywith all statutory and regulatory obligations and relevant practicepolicies/guidelines, including information governance and confidentiality
* Maintainquality within the practice and alert other team members to issues of qualityand risk
* Participatein the development of nursing policies and protocols with reference to bestpractice

* Toattend and contribute to practice and clinical meetings

Thepost-holder will implement and lead on a full range of promotion and managementof their own and others health and safety and infection control as defined inthe practice health & safety policy, the practice health & safetymanual, and the practice infection control policy and published procedures.This will include (but will not be limited to):
* Usingpersonal security systems within the workplace according to practice guidelines
