Apollo Primary Care Network (PCN) is excited to offer a fantastic opportunity for an experienced and dynamic Advanced Clinical Practitioner (ACP) to join our thriving Multi-Disciplinary Team (MDT). We have built excellent working relationships across our five GP practices and with colleagues from other PCNs, creating a collaborative and supportive environment. We welcome applications from ACPs with experience in Nursing, Paramedic, or Clinical Pharmacist specialisms to contribute to the delivery of high-quality, patient-centered care across our network.
Main duties of the job
As an Advanced Clinical Practitioner, you will be a key member of our MDT and will play a vital role in delivering the PCN DES (Directed Enhanced Service) and improving capacity and access for our patients. Key Responsibilities:
1. First Point of Contact: Assess and manage a broad range of patient conditions across all ages, using triage models, including online triage, face-to-face consultations, and home visits.
2. Direct Patient Access: Provide direct access for patients, planning investigations, treating, and referring as appropriate for undifferentiated and undiagnosed health issues.
3. Professional Autonomy: Make clinically autonomous decisions, including physical assessments, prescribing, and managing patient care plans based on evidence-based guidelines.
4. Achieving IIF Targets: Contribute to achieving IIF (Improvement and Impact Fund) targets as part of the Network Contract DES, focusing on quality care and patient outcomes.
5. Patient Record Management: Maintain accurate and up-to-date patient records, storing details of referrals and care pathways.
6. Collaboration: Develop and maintain effective working relationships with key stakeholders, including GPs, consultants, district nurses, social services, Trusts, and the voluntary sector.

This is an exciting opportunity to engage in a diverse range of activities, including online triage, face-to-face consultations, and home visits. You will be expected to make autonomous professional decisions, applying your knowledge and skills in physical examination, applied pharmacology, and evidence-based prescribing.
Key Responsibilities:
1. First Point of Contact: Assess and manage a broad range of patient conditions across all ages, using triage models, including online triage, face-to-face consultations, and home visits.
2. Direct Patient Access: Provide direct access for patients, planning investigations, treating, and referring as appropriate for undifferentiated and undiagnosed health issues.
3. Collaboration with the Digital Team: Work with the PCNs digital team to evaluate and implement various IT and digital solutions to enhance patient care.
4. Professional Autonomy: Make clinically autonomous decisions, including physical assessments, prescribing, and managing patient care plans based on evidence-based guidelines.
5. Achieving IIF Targets: Contribute to achieving IIF (Improvement and Impact Fund) targets as part of the Network Contract DES, focusing on quality care and patient outcomes.
6. Patient Record Management: Maintain accurate and up-to-date patient records, storing details of referrals and care pathways.
7. Training and Development: Assist with the delivery of training to colleagues within and outside the profession, in consultation with GPs, the nursing team, and other primary healthcare professionals.
8. Protocol Development: Regularly review and update PCN protocols and guidelines to ensure best practice.
9. Collaboration: Develop and maintain effective working relationships with key stakeholders, including GPs, consultants, district nurses, social services, Trusts, and the voluntary sector.
Person Specification
Experience
Essential
1. At least 1 year experience post qualification
2. Driving License to travel to Care Homes/other locations across the PCN if required
3. Must be eligible to work in the UK
Desirable
1. Experience of working in community/General practice/Primary care
2. Knowledge of PCN IT systems/ IT proficient (EMIS)
3. Teaching/Assessment and mentoring qualification; Leadership course Experience of working with Frailty or similar community services Demonstrates understanding of relevant national strategy/policy and how to relate this to the service
