Look no further Defence Primary Healthcare (DPHC) are looking for a dynamic, organised and efficient Band 5 Practice Nurse who will be working within our primary healthcare team at Leuchars Station, St. Andrews. Leuchars Medical Centre is due combine with Condor Medical Centre. On occasion within your planned working schedule, you may be requested to work at Condor Medical Centre, however, in these situations, travel time and cost will be allowed for. This position is part time and offers 15 hours per week. 5 hours shifts to be worked over the week between Monday and Friday. Does this sound like the role for you? Job description The successful candidate will be undertaking a range of nursing assessments, working independently and providing appropriate care/treatment in conjunction with General Practitioners (GPs) and other medical staff. Care will be appropriate to need and in accordance with Ministry of Defence (MOD) and DPHC policy. The Practice Nurse will strive to improve standards and be involved in determining the practice population needs. These will be focused on patient experience and best practice. The Practice Nurse will have training and clinical supervision responsibility for junior members of the multidisciplinary team. They will be involved with communication with outside agencies to provide a range of clinical and management services to the Medical Centre population at risk (PAR). The Practice Nurse will adhere to the Nursing & Midwifery Council (NMC) Code of Professional Conduct at all times. Person specification Main details of the role include but are not limited to: Prioritising health problems and intervene appropriately in order to assist the patient in complex, urgent or emergency situations. Assessing, planning, implementing and evaluating treatment programs that promote health and wellbeing. Proactively identifying and managing treatment plans for patients at risk of developing a long-term condition as appropriate. Performing Healthcare Governance activities as dictated by Line Management in accordance with the facility Healthcare Assurance Framework (HAF) document. Licences Registered nurse with current, unrestricted registration on Nursing & Midwifery Council Register. Qualifications Degree, diploma or appropriate training as recognised by professional body (NMC/HCPC etc) in a relevant area. In-date Basic Life Support (BLS) and Automated External Defibrillator (AED) qualification. Immunisation Trained. Child Protection Level 2.
