Job overview
Applications are invited for a Specialty Doctor in Acute Frailty (0.4 WTE). The post is substantive. The successful candidate will join an established multidisciplinary team supporting an expanding Frailty service delivering excellent clinical care for the frail population of Mid Cheshire.
Main duties of the job
The successful candidate will be based on the Frail Elderly Assessment Unit (FEAU) carrying out comprehensive geriatric assessments of frail elderly patients. There will be participation in twice weekly MDT meetings for the 30 “bed” virtual frailty ward.
The successful candidate will be expected to carry out assessments of patients at risk of admission via ‘hot clinics’ to ensure high quality care is delivered in the right place.

Qualifications
* MBBS or equivalent medical qualification
* Evidence of foundation competencies
* Evidence of achievement of ST1 & ST2 competences (or equivalent) in a relevant specialty

* Acute care safe: up-to-date ALS. Out of hours experience relevant to the job.
* Clinical judgement: experience in making clinical decisions and managing risk. Knows when to seek help, able to prioritise clinical need.
* Relevant specialty clinical knowledge: capacity to apply sound clinical knowledge relevant to the job, specialty knowledge exam.
* Practical skills: shows aptitude for practical skills required in the job. Proven ability to work effectively in different clinical settings required in the job.
Fitness to Practise
* Full GMC Registration with a Licence to Practise

* Comprehensive Geriatric Assessment: understands the components of a comprehensive geriatric assessment and is confident in initiating the assessment in a frail person.
* Advanced Care Planning: has the ability to recognise patients who would benefit from advanced care planning and end of life care discussions and is able to carry out these discussions with patient and families.
* Supervision: ability to supervise junior team members and assist in conducting audits/QIPs.
