We provide services and supports to adults and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection, and referral services.
About the role
We are seeking a values-driven and proactive Case Manager! You will provide case management support to assist people to achieve housing stability.
Reporting to the Team Leader, this is a fixed-term part-time position, 9 days a fortnight, contracted for one year, based in Stafford, QLD. Salary and conditions are in accordance with Social Community Home and Disability Services Award Crisis Level 2.
You will successfully
* Establish effective relationships with clients that encourage self-determination and participation
* Provide case management support to individuals in accordance with TSA case management models and standards of practice
* Complete thorough assessments, inclusive of needs, risks, and vulnerabilities
* Conduct ongoing assessment of the individuals’ presenting issues, vulnerabilities, and needs from initial intake to exit
* Develop individual case plans in collaboration with clients that include a range of interventions addressing the needs or goals of clients
* Implement case plans with active client participation
* Assist clients to access support services and establish community supports
* Display high levels of emotional intelligence, with an awareness of how personal values and beliefs can impact work
* Participate in regular supervision and case review meetings
You will have
* Tertiary qualification in Community Services, Social Work or related field. Degree level qualification preferred
* Relevant experience in a social service environment including in client assessment and case management
* A QLD Blue Card (Working with Children's Check)
* A current QLD Driver's Licence
