Job summary This is an exciting opportunity for the right experienced applicant to take up the role of PCN Clinical Lead across Primary Care Networks to support GPs and the management of mental health issues (Adult SMI) in primary care settings across West and South Norfolk, making the routes into and through mental health services clearer and easier for everyone to understand. The role will provide an opportunity for better joint working across primary care and adult secondary care mental health systems ensuring the whole system is focused on working in an integrated way to care for patients and will make it easier for people to obtain the help they need. As part of the responsibilities of this role, you will manage a team of specialist Interface Pharmacists and Psychology staff, and Mental Health Practitioners embedded within Primary Care. Main duties of the job To fully support the West and Central Locality with the strategic and operational planning and delivery for their designated specialist service area(s) within the locality. You will be responsible for ensuring the effective management of the day-to-day operational / clinical issues arising within mental health service delivery. To build and maintain effective relationships with key stakeholders and partners across the integrated system to provide safer, kinder, and better services. To undertake clinical activity as necessary, especially complex management referrals. To actively monitor performance, quality, safety and service development within their service areas, in line with locality and Trust-wide policies and protocols.
