Are you a passionate and caring nurse, with a keen interest in haemoglobinopathies? We are looking to recruit a talented and empathetic individual to grow our team by working jointly with our specialist teams and community services, in serving our sickle cell and thalassaemia population across our borough.

Main duties of the job
Professional/ Clinical responsibilities
* Empower patients and carers by providing health education enabling informed decisions about their care.
* Enhancing compliance with preventative actions such as vaccinations and prophylactic medications.
* Provide guidance on self-management to prevent exacerbation of the patient’s condition.
* Address social and lifestyle factors which impact condition management and overall quality of life.
* Act as a patient advocate through the application of ethical, legal, and professional knowledge and skills.
* Pre-contact with patients to reduce DNA rates to clinic appointments.
* Enable implementation of individual care plans for patients in all settings.
* Advise on appropriate drug management at home, including dosages and information on non-pharmacological management techniques.
* Provide in-reach service to secondary care.
* Promote good clinical practice in line with evidence-based guidelines and policies.
Person specification
Training and qualifications
* First level Registered Adult Nurse with current NMC Registration
* SSSA Practice Assessor certificate or equivalent (or willingness to undertake training)
* Phlebotomy (or willingness to undertake relevant training)
* Evidence of continued professional development
* Haemoglobinopathies Advancing Client centred care course
Experience
* Knowledge of sickle cell disease and thalassaemia
* Evidence of multidisciplinary and multiagency working
* Experience of safeguarding adults
* Experience of working in the community
Specific Skills
* Specialist knowledge of the medical, nursing, social and psychological implications of sickle cell and thalassaemia
* Evidence of teaching skills, in particular teaching patients in how to manage their conditions
* Excellent verbal, written, interpersonal and communication skills.
* Time management and organisation skills.
* Sound understanding of the National agenda for haemoglobinopathies.
