Our Collaborative Care Team (CCT) is offering one permanent role and one fixed-term position until January 2026, in an innovative organisation that’s truly changing the face of primary care. What You'll Do:
As an Occupational Therapist in CCT, you’ll create personalised care plans and conduct holistic home assessments, empowering adults with complex needs to live independently. You’ll play an essential role in our fresh, forward-thinking approach to primary care.
What We’re Looking For:
* Qualifications are a must (Registered Occupational Therapist)
* Experience working with service users with complex needs, including mental health
* Strong communication and professional reasoning skills
* The ability to handle confidentiality with sensitivity
