Hours per week: 08.00-17.30
Location: Community based (you might be required to work night shifts such as shadowing shifts, sign off’s, spot checks), WFH, 1 day a week in Liverpool St office preferably on Tuesday’s
WE ARE LOOKING FOR A NURSE IN THE LONDON AREA AND ONE IN HERTFORDSHIRE
Aim of the role
* Conducting initial assessments of service users, develop individualized care plans in collaboration with patients, families, and healthcare professionals.
* Ongoing care plan reviews and risk assessments
* Observing care staff and ensuring their competency
* Managing clinical paperwork, including Medication Administration Records (MARs) – monthly audit
* Providing general clinical oversight of the branch and service users
* Occasionally providing direct care to service users to maintain hands-on clinical skills (e.g. tracheostomy change / sign off carer competency at same time)
Main duties:
* Shadowing & sign off competencies for candidates
* Initial assessments & implement care plan for individual service users
* Care plan & RA reviews – shared task between Community and Clinical lead nurse
* Medication audit – monthly, via Birdie
* Quarterly spot checks
* Building rapport / regular check in with parents – Establishing effective working relationships with parents, their families and carers. This will include promoting individual rights and recognising and respecting their contributions to care planning and delivery.
* Working with multi – disciplinary team
* Ensuring admin is up to date
* Working closely with the Clinical Lead nurse to ensure care delivery is of a high quality and meets required standards
* Support and Deputise for the Clinical lead Nurse and provide knowledge and skills to support care delivery.
