Are you an experience District Nurse with a Practice Assessor qualification looking for a new challenge in North Lincs?
As an experienced District Nurse with a Level 7 Practice Educator qualification you will be delivering advanced, high quality clinical interventions
to your patients whilst supporting the education and training of future District Nurses workforce.. The Community Practice Assessor holds a
pivotal role in teaching, facilitating learning, the specific assessment of District Nursing students and all staff within the District Nursing Service to
ensure they have the right knowledge and skills who are safe and fit for practice; providing assurance the workforce will deliver the high quality of
care to meet the needs of the population including patients with continuing healthcare needs, health coaching and well-being. The Practice
Assessor will conduct evidenced based assessment of students in line with the HEI curriculum and NMC standards, also providing constructive
feedback working alongside the academic assessor.
The post holder will demonstrate skills commensurate with their level of practice across all four pillars of practice. In addition to their clinical and
teaching responsibilities they will also act as role models in terms of leadership and quality improvement culture. Working with multiple partners
within and without the organization the post holder will promote, support and develop our community nursing workforce including specialist
community clinicians in line with NMC requirements and standards for the Specialist Practice Qualification.

This posts will provide the opportunity to take forward the expanding role of community nursing within the multi-disciplinary integrated team to meet the long term conditions and end of life care agenda, linking and working closely with the GP’s. It will be expected that you will have an understanding and commitment to the continuing development and modernisation of community nursing and must have effective communication and negotiation skills.
The post is suitable for a newly qualified District Nurse, who will gain caseload management skills and experience over a 6 month preceptorship period.

Facilitation of Learning
Learning Teaching and Assessment
• Design, plan, implement and evaluate learning and development programmes for a variety of staff groups both registered and non
registered.
• Provide advice and support to other practitioners
• Engage with education providers to contribute to curriculum development and delivery
• Take a lead role in ensuring the application of standards and guidelines that underpin a quality learning experience e.g. mentorship and
practice learning standards
• Act as the education lead for the team by demonstrating effective management, clinical leadership and professional/clinical guidance to
the District Nursing Network.
• Be responsible for updating and maintaining the ‘Practice Assessment Record and Evaluation’ system (PARE)
• Triangulate education audits with the HEI and Quality Improvement Nurse
• Assess and/or supervise the clinical knowledge and skills of student District Nurses during their practice placement and all other staff
within the team including staff new to community.
• Offer preceptorship to newly qualified specialist nurses.
• Ensure all staff have the requisite competencies to work as a community nurse/district nurse and monitor the compliance of those
competencies to maintain quality and safety.
Create a learning Environment
• Analyse the range of factors which influence learning and take account of these when planning and managing learning experiences
• Build capability and capacity to support learning in practice settings
• Create a positive learning culture through assuring and improving the quality of educational opportunities within the work environment
Leadership
Teamwork and Development
• Provide strong and effective leadership across professional and organisational teams/boundaries
• Critically review skill mix, apply workload assessment tools and use results to build a business case- work with team lead

• Work collaboratively with the Team Lead to monitor the delivery of high quality person-centred care by the team and act on the results
• Provide timely feedback to team that recognises good performance and identify areas for improvement
• Review training and development compliance of all staff, ensuring all staff have a personal development plan. Support staff in meeting
their development needs and reaching their full potential as identified in their PADR.
• Undertaken annual PADR’s maintaining the C&T standards
• Ensure you are knowledgeable regarding Human Resource (HR) policies in relation to sickness absence and conducting return to work
interviews for staff.
• Develop and support the induction and preceptorship of staff new to role/community working together with practice development and
quality improvement
Professional and organizational leadership
• Ensure staff rotas cover the service by populating them using e-roster.
• Lead and monitor clinical effectiveness and efficiency to enhance management of resources
• To ensure specific service areas deliver a financial performance in line with the agreed financial plan continuously exploring and
implementing all opportunities for cost improvement.
• Actively encourage involvement of service users to influence and improve person-centred care
• Work in conjunction with the Team Lead in the development, delivery, monitoring and evaluation of standards of care and act on the
results
• Lead innovation and quality improvement and promote involvement of others
• Work with community nursing teams and the district nursing management team to support them to lead change.
• Identify poor performance and take responsibility for ensuring appropriate development opportunities are available to address concerns
