Job summary We are looking for an enthusiastic, motivated, andcompassionate individual to join us in the Community Stroke Team in NottinghamCity. The CityCare community stroke team is a large multi-disciplinary teamthat consists of specialist nurses, therapists and support staff. The team actswithin CityCares organisational values and behaviours making for a greatworking environment. You will be supporting individuals, their families andcarers in their own homes and other community settings such as care homes. Thisis a people focussed role, seeing clients from within Nottingham Citys diversepopulation. This includes assisting patients who are attending rehabilitationgroups such as Stroke Ability and Chat Ability. The role itself underpins the service by carrying outinterventions and care plans established by specialist nurses, therapists andadditional clinical staff. Competency based training is a requirement of therole to support the successful candidate. The service operates 7 days a week so some weekend work on arota basis is a requirement of the post.However, this includes daytime work only. Main duties of the job Thepost holder will be involved in providing rehabilitation by following goals setby clinicians within the team to optimise the patients level ofindependence. The Rehabilitation SupportWorker will promote independence using skills from Occupational Therapy,Physiotherapy, Speech and Language Therapy and Nursing, following acompetency-based framework. Abackground of caring for people is essential either in an informal capacity orin a paid role. In addition to clinical skills, you will need to demonstrateexcellent interpersonal and communication skills. You must be able toprioritise tasks and manage your time effectively.
