The post holder will focus on medicines optimisation in Mental Health. They will liaise with all Sheffield Accountable Care Partnership organisations to: Support the development and work with existing pathways, protocols and guidelines so the patients receive safe, clinically effective and timely mental health medication in the most appropriate care setting. Work with health and social care providers (such as, but not limited to; SCCG, SHSC, STHFT, PCS / GP practices, SCHFT and SCC) to promote existing and develop new tools, to increase the annual health checks in patients with Learning Disability and Severe Mental Illness and other Mental Health conditions as indicated. Promote and deliver the principles of STOMP/ STAMP to reduce the prescribing of psychotropic medication in patients with LD. Autism or both, with an aim of reducing premature mortality and morbidity in patients in these patient groups. Promote non-pharmacological support to all key stakeholders to maximise the outcomes for the patient and to complement and, where clinically appropriate, replace the need for prescribing psychotropic medication. Provide support, clinical expertise and educational sessions to pharmacy / HCP / patients in primary care networks (e.g., Community Pharmacy, Primary Care Sheffield and practice-based pharmacists). Support the transition of patients across care settings so they receive the required level of care to support them in taking their medication safely in the most appropriate care setting. Provide expert advice around prescribing in Mental Health conditions to primary care organisations The post holder will work closely with identified Primary Care Networks Utilise local intelligence and data to target areas of Mental Health Prescribing to audit and review. Produce action plans and drive implementation changes as indicated by results. To share learning across the ACP footprint as appropriate Work with SHSC and GP practices to support smooth transition of complex patients, from specialist to primary care, and vice versa as needed. Support the care of patients, with regard to medicines, when they transition from childrens to adult services / primary care. Work to and embed STOMP / STAMP principles Support the management of shared care so patients are being monitored and reviewed in line with best practice / evidence / guidance. Carry out medication reviews in patients / patient groups identified as being a priority within the primary care setting (e.g. via audit / concerns/ national guidelines) General requirements The post holder will: Develop a culture that promotes equality and values diversity. Work as an Independent prescriber or work towards this qualification. Form part of the mental health clinical pharmacy team ensuring the safe effective use of medicines throughout the ACP. Participation in continuous professional development Participation in clinical supervision Spend significant amounts of time on VDU if necessary to meet objectives/deadlines. Managerial Responsibilities Able to take action based on own interpretation of broad professional policies/procedures where necessary. Able to handle clinical queries and prioritise tasks according to level of urgency. Able to make decisions and taking urgent actions in isolation from pharmacy work colleagues as needed. Principal duties The following represent a broad outline of tasks the post holder will be expected to perform in achieving their key result areas. These are not intended as a fixed list of duties. The post holder will, under the direction of the Chief Pharmacist, perform whatever duties are reasonably necessary to achieve the job purpose and the overall safe provision of pharmacy services throughout the Trust. System-wide responsibilities Promote medicines optimisation in Mental Health across the ACP and to promote tools and resources to maximise wellbeing. The post holder will; Work with pharmacists within the ACP to promote current prescribing guidelines and identify areas where guidelines are needed to support the safe, clinically appropriate prescribing of medication to patients with mental illness to enable patients to access medication and review in the most appropriate care setting. Consider what actions / tools and resources are needed to embed guidelines within all relevant organisations. Use local intelligence, feedback, national priorities and local data sources to identify prescribing areas to review and work with key stakeholders to develop audit tools to promote best practice around medicines optimisation in MH. Promote the importance and increase the uptake of monitoring / annual health checks in patients with SMI, LD other MH conditions Identify gaps / opportunities and work to find solutions to further optimise the use of medication in mental health, such as training needs, clinical review templates, best practice guidelines, SOPs Work with key stakeholders to develop and deliver education and training around prescribing in MH, targeting, but not limited to; community pharmacy, Primary Care Sheffield, GP practice staff, patients. Have an understanding of non-pharmacological support (psychological and social) on offer to support mental health and wellbeing, and promote when these can be used to support patients so that prescribing is optimised and patients are empowered with a range of tools to get /keep well. Promote the principles of STOMP / STAMP to reduce inappropriate prescribing of psychotropic medication and to improve the morbidity and mortality of people with LD, autism or both. Work to support the transition of patients as they move across different care settings so that their medicines needs are met and monitored safely in the most appropriate care setting. The post holder will work with the current Medicines Optimisation Team (CCG) to respond to queries from the CCG membership practices around prescribing in MH. Practice based responsibilities Work with designated Primary Care Networks to Help improve medicines optimisation and medicines management (within primary care networks) for patients who have mental health related conditions. Use data, local and national intelligence to identify areas where improvement can be made around medicines optimisation in MH. Lead on implementing and delivering change to improve outcomes for patients. Be aware of local and national guidelines and formularies and implement these into practice. Provide patient-facing, clinically focused medicines optimisation reviews that will help patients get the best out of their medicines and assist in improving service and cost efficiencies. This may in turn help reduce GP related / secondary care appointments related to mental health conditions. Participate in multidisciplinary team meetings and medication reviews. Communicate to and receive complex medicines information from prescribers, patients and carers including those with language difficulties or learning disabilities, around prescribing in MH. Help reduce polypharmacy, inappropriate prescribing and help to improve monitoring of prescribing MH medication in accordance with NICE and local guidelines. Support practices with the delivery of MH related prescribing QoF indicators and CQC requirements. Support the transition of patients as they move between different care settings, e.g. from SHSC to primary care. Make sure robust systems and processes are in place to optimise care around medicines and patients are managed safely in the most appropriate care setting. Embed STOMP/STAMP principles, using audit, review and education, linking with relevant health and social care providers as needed. Manage systems and processes around shared care arrangements to support safe prescribing in Mental Health To be a role model and resource for other health care professionals, e.g. practice pharmacists and technicians around medicines optimisation in MH. Develop collaborative partnerships across organisational boundaries. Provide specialist advice and information on the safe, cost-effective and evidenced based use of medicines in mental health and other therapeutic areas for staff. Deliver education and training packages around prescribing in MH to other disciplines, including patients, as identified / needed. To participate in the training and supervision of pharmacy undergraduates, as requested. Plan and organise own timetable and workload including medication reviews, training sessions, audit schedules and project work. Information resources Utilise a range of IT systems including SystmOne, Emis Web, Eclipse Live, Insight, Ulysses, DATIX, ICE (to access pathology results), JAC/CMM and Microsoft Office to enter information and produce reports /training. Provide reactive and pro-active medicines information to doctors, nurses and other health care professional staff. Research and development Undertake surveys/audits of the Clinical Pharmacy service and medicines use and participate in medicine related audits in primacy care. Produce reports and action plans for audits undertaken. Support any clinical trials subscribed to by the trust/ACP.
