This post holder will be responsible for ongoing management of patients receiving nutrition
support both inpatients and outpatients. The main duties are supporting the lead nurse
with the care of inpatients receiving parenteral and enteral nutrition, and for the ongoing
delivery and care of home parenteral nutrition and high output stoma clinics.
Support ward staff with the management of intestinal failure, parenteral nutrition,
and high output stoma on the Colorectal Unit
Assess and educate patients in the home nutrition and high output stoma clinic
which runs 5 sessions per week.
Conduct telephone and attendance clinics for patients discharged with nutrition
support and facilitate access to the Colorectal Unit and Surgical Emergency Unit to
support a reduction in readmissions and improve patient experience
