Job Description
Qualified Social Worker, Derby City Council, Adult Hospital to Home Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team.
The team works extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager.
Responsibilities include:
1. Work within the discharge to assess model working with customers within the hospital and the pathways.
2. Collaborate with the Home First and Occupational therapy team and closely with Carelink & Telecare, LAC, Locality teams, and Brokerage. Complete assessments in all 3 pathways with the aim of returning customers to their homes.
3. Be responsive and able to work on your own initiative, have experience of assessing and reviewing customers, with knowledge of equipment and a re-ablement ethos. Attend Multi Disciplinary meetings and possess excellent communication, IT, and organizational skills.
4. Travel across the City of Derby; must have a full UK driving licence, use of a car for work purposes, and business use insurance or alternative transport.
5. Liaise with other professionals.
6. Work 36 hours per week (flexible by negotiation).
7. 2 Month Temporary Contract. Start date subject to DBS and reference clearance.

The successful candidate will need to hold a DipSW or equivalent, must be registered with the HCPC, have a current DBS, and be a car driver.
