Job summary
An exciting opportunity has now arisen within the Manchester Community/District Nursing service here in south Manchester for a Registered Nurse associate (NMC Nurse Associate registration is essential) or Band 4 Assistant Practioner .
Working within the Community Nursing Team and contributing to the provision of a coordinated and comprehensive service for patients receiving community Nursing interventions including patients resident in care homes in South Manchester.
The post holder will work as part of the Integrated Care team and neighborhood. The team aims to provide effective supportive Nursing interventions to patients with a nursing need requirement in their residential setting .
Main duties of the job
The post holder will be part of a community /District Nursing team and will work alongside other health professionals in the implementation, delivery and evaluation of Nursing interventions, there will be opportunities to promote independence and encourage improved health, functional capabilities and quality of life.
You'll also need to demonstrate how you will meet the ability to be mobile within community.
