The Community Stroke Rehabilitation Team is a nationally respected Early Supported Discharge Team. It is an essential part of the Portsmouth Hospitals Stroke Pathway, ensuring the timely transfer of patients out of hospital for rehabilitation at home.
An opportunity has arisen within CSRT for a Senior Healthcare Worker to join the team. The post holder will be closely working with a large MDT (multi-disciplinary teams) with competencies across several disciplines. You should have the willingness and ability to learn new skills as working across MDT is essential, as well as being an excellent communicator is vital.
Your role will consist of supporting stroke patients in their home with personal care and other activities of daily living, as well as carrying out therapy tasks under direction from the wider MDT.
Experience of working with patients with neurological deficits and community working would be desirable. You need to be a confident car driver and user, as the team covers a large geographical area.

Key Responsibilities:
Following care and therapy plans provided by registered healthcare professionals, you will be assisting the team to provide stroke rehabilitation to patients within their homes. This will include providing essential care in a therapeutic manner, as well as support with adjustment to life after stroke and reintegration in community. In addition, you will take all possible steps to safeguard the welfare, safety and security of patients, their families and other staff.
Qualifications
Essential
* QCF or BTEC or equivalent (NVQ) Level 2 qualification in Health and Social Care/Clinical Health, or completion of NHSP Care Support Worker Development Programme.
* English and Maths at Level 2 Functional Skills or GCSE equivalent.
* Full driving license with appropriate car insurance.
* Desire to deliver compassionate hands-on rehabilitation and care in patients' homes.

* Ability to communicate verbal and written English.
* Ability to work independently demonstrating initiative, decision-making and prioritisation skills.
