Job Description for Clinical Pharmacist (IP) at The Pharmacist Network
Main Responsibilities:
* Provide expert advice on medications to clinicians and patients.
* Deliver clinical leadership on medicines optimization and quality improvement.
* Ensure appropriate and efficient use of medication.
* Educate patients on medication and disease prevention.
* Conduct medical-related audits and work on service improvement with the practice.
* Medicine reconciliation and review post-discharge.
* Support practice staff with prescription and medication queries.
* Improve patient outcomes and ensure better access to healthcare.
About the Role:
We are seeking an experienced Clinical Pharmacist (IP) to work full-time onsite at Leyland Practice within Lancashire. The ideal candidate will have a minimum of 12 months' experience working in general practice/primary care, a relevant degree/qualification, and independent prescribing qualification.
The successful candidate will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and GP's. They will help resolve medication-related queries, work as part of a multidisciplinary team, and improve patient outcomes.

Requirements:
* A minimum of 12 months' experience working in general practice/primary care.
* A relevant degree/qualification and registration with GPHC.
* Independent prescribing qualification.
* Experience with EMIS or System 1.
* Desirable to be on the CPPE Primary Care pathway or willing to partake.
* Able to commute to onsite location/live within close proximity of practice.
* Excellent communication and interpersonal skills.
