To be responsible for the provision of paramedical services, working as part of the practice's multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The paramedic practitioner will be required to work autonomously in the practice and in the community (home visits), providing a range of services such as assessment, diagnosis, treatment, telephone triage and referring patients appropriately, as well as supporting the management team in the reviewing of clinical policy and procedure.
Main duties of the job
1. To support our patients using an on the day telephone triage system to offer call backs, face to face appointments and signpost as appropriate.
2. To attend to vulnerable housebound patients in need of care on the day at their home or care setting.
3. To support the practice with reviews and assessments both in the practice and our housebound/care setting patients.
4. To liaise with our care homes, district nurses, and other care frameworks to support patient care.
5. Support clinicians with medical report requests if required.
6. Promote and partake in practice campaigns which may include vaccinations.
About us
Bedford Street Surgery is a two-site practice located in Bletchley, Milton Keynes with a branch site in Furzton, Milton Keynes. We currently have over 15,000 patients across both sites.
We have a well-established clinical MDT team consisting of paramedics, prescribing nurse practitioners, respiratory nurses, health care assistants, practice nurses, and GPs who are supported by our management and admin team of secretaries, care navigators, and data quality.

Job responsibilities
1. To be responsible for the provision of paramedical services, working as part of the practice's multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The paramedic practitioner will be required to work autonomously in the practice and in the community (home visits).
2. To oversee the triage board on a day-to-day basis to provide patients access to a clinician for on-the-day advice. Oversee our housebound patients by visiting at their request but also completing any essential care checks with them.
3. Liaise with both external sources to provide help and support to patients and also work with the staff within our network and practice as part of a multidisciplinary team.
4. Work alongside our clinicians to provide on-the-day care and support our clinicians with regular reviews, assessments, and referrals.
5. To attend meetings as part of a multidisciplinary team and palliative team to offer shared care.
6. Provide a range of services to include assessments, diagnosis, treatment, telephone triage, and referring patients appropriately as well as supporting the management team in the reviewing of clinical policy and procedure.
Person Specification
Licence
* Must have their own car and a clean licence.
Qualifications
* Registered Paramedic (HCPC).
* Postgraduate diploma or degree (Primary Care).
Experience
* Experience in triage.
* Experience of dealing with a range of clinical conditions.
* Experience of working in a primary care environment.
* Experience of working as a paramedic practitioner.
