We are currently looking for a Registered Adult Nurse or Physiotherapist to work within our SPOC / In-reach team. The post holder will have access to multi-organisational partners to improve patient experience and will support patient flow to contribute to timely and excellent standards of care.
Working across the pathway will give the post holder significant exposure and experience to the workings of the NHS system.
If you have experience of working in acute, community or rehab services dealing with complex challenges and are keen to do something different in a highly performing, stimulating and fast-paced environment, then this may be your opportunity.
You will be fully supported by the team and service manager as you grow your knowledge of clinical services within West Hertfordshire and beyond to ensure patients receive the most appropriate care in a timely manner.
Main Duties of the Job
The post will work alongside an established and friendly SPOC/In-reach team which combines clinical triage skills with operational working at the system level to enable patient flow from acute hospitals.
The successful candidate will perform clinical triage to determine suitability for bedded unit rehab pathway in West Hertfordshire whilst also signposting referrals for pathway 1.

Person Specification
Education/Qualification
* Degree in Physiotherapy or Adult Nursing
* Current HCPC or NMC registration
* Demonstrable successful innovation in clinical practice
* Evidence of CPD relevant to acute and rehabilitation practice
* Evidence of service improvement within previous role
* Diploma in Management Studies or equivalent / Postgraduate qualification in specialist area.
Experience
* Previous experience working within either an acute hospital or rehab unit or community service
* Able to communicate with patients, relatives and MDT and on occasions deal with conflicting views
* Appropriate clinical experience post registration to work at senior level
* Experience working in a multi-disciplinary team
* NHS experience
* Knowledge of common causes of delays which lead to a delayed transfer of care (DTOC) for patients
Skills and Knowledge
* Communicate effectively at all levels
* Well-developed verbal and written communication skills
