Job Overview
The successful candidate will hold a professional qualification of OT, Social worker or RMN, under the line management and supervision from within the integrated mental health team.
The post holder will assist in the overall delivery of care in a care coordination role to patients within the mental health team at HMP Risley and HMP Thorncross. You will be responsible for the assessment of care needs and the development, implementation, and evaluation of programmes of care for mental health.
This includes respecting individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.
The successful applicant is required to demonstrate a “can-do” attitude, be resilient and possess excellent organisational skills. An ability to work well as part of a multidisciplinary team is essential.
Healthcare services operate 7 days a week, and the successful applicants would need to be flexible in their approach to meeting the demands of the service.
The successful candidate will work towards providing the highest standard of care for the prison population ensuring consistent and effective nursing practice as well as operational activity.
The days within the prison service are varied and there is a wide range of training and development opportunities available.
Main Duties of the Job
* Be a full member of the multi-disciplinary team (MDT). Effectively manage own caseload.
* Undertake formal mental health and social care assessments of new clients.
* Undertake assessments of clients as part of the integrated mental health duty system in accordance with agreed operational procedures.
* Assess, care-plan, provide interventions and evaluate and record mental health care delivered to clients on own caseload.
* Provide effective care coordination to an identified caseload of complex service users, ensuring the delivery of evidence-based interventions to encourage successful outcomes under the Care.
* Provide Specialist Mental Health Assessments, Continuing care Assessments and advise management plans as requested to promote positive mental health and physical care to clients in all situations.
* Encourage and support clients to collaborate in their treatment programme and recovery.
* Encourage self-help as much as possible, promoting the maintenance of skills and independence in clients.
* Undertake specialist assessments when required in line with the Mental Capacity Act 2007, The Mental Health Act and Local Authority Safeguarding Adults Procedures.
* Undertake and record comprehensive risk assessments, devise and implement risk management plans for clients at risk of vulnerability, self-harm, or risk of physical, sexual, or emotional harm to others, using Risk Assessment Tool.

* Pay Enhancements – 30% additional for Evenings (8pm onwards) and Saturdays and 60% additional for Sundays and Bank holidays.
* 27 days annual leave plus bank holidays rising to 29 after 5 years and 33 days after 10 years.
* Must have lived in the UK for over three years.
* At least 2 years experience working in mental health.
* Have worked in secure services.
* Able to work autonomously.
* Knowledge of step care model.
* Able to supervise others.
* Able to complete bloods and ECG.
