The post holder will, through a range of defined competencies, acquire all of the skills necessary to progress into the role of care coordinator. This will include a requirement to practice care coordination skills in the clinical setting. To seek out and engage in management and clinical supervision with senior clinicians and clinical team managers. To establish priorities in workload in partnership with senior team members and ensure effective use of time and resources.
To maintain a commitment to person-centred care and to promote the use of the recovery model where appropriate. To support and carry out comprehensive holistic mental health assessments, formulation, safety plans, the planning and implementation of appropriate care and outcome measures. To act as a positive role model, providing support to other team members. You will also be expected to employ leadership within your role and will provide clinical supervision, preceptorship, and support to junior colleagues. Student education and support is inherent to this role.
