Job summary An exciting opportunity has arisen for a proactive, dynamic and inspiring Band 6 Physiotherapist to join Taunton & West Somerset Community Rehab Service. Working with people on their identified rehabilitation goals to reach their maximum potential and personal health & well-being in their own homes or local communities. You will be working alongside OTs, Rehab Assistants/Practitioners and Pharmacy Tech and liaising with other health, social and voluntary services. You will be supervising other staff and participating in the Duty rota. We run an in-service training programme and continually aspire to improve the service we deliver. Main duties of the job The main aim of the Community Rehab Service is to enable patients to work towards their own identified goals in their own environments. The post holder will be required to use enhanced knowledge and skills to assess and facilitate rehabilitation programmes with patients and their support network to maximise their daily abilities in their daily life and ongoing self management skills. There will be a requirement for post holders to work closely with colleagues in the local neighbourhood and community services/teams. You will be expected to carry out specific therapy assessments and devise rehabilitation programmes. This will include treatment planning, assessing risk, promoting and enabling independent functioning.
