Job overview Are you a recently or almost qualified Occupational Therapist who would like to work in a community mental health team? South Shropshire Mental health Service is looking to recruit a Band 5 Occupational Therapist into a developmental role that following completion of preceptorship would lead to a Band 6 role within the team. This is a varied role that will allow the post holder to develop the key skills required for working in a busy rural community team whilst offering supervision and preceptorship to allow the successful applicant to gradually gain confidence and skills. The post will be primarily based in the South West Shropshire area with the main base being Ludlow Community Team but there may be requests initially to gain experience from Occupational Therapy team members within the Shrewsbury Teams and occasional assessment requests to support the South East part of the team, although all case work and interventions will be within the South west area of Shropshire. Main duties of the job · Take responsibility for the management of a caseload which will increase in complexity as you develop, which includes specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans (Eligible for care and Patient Knows Best). · Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy. · Assess patient's individual care needs, develop, implement and evaluate programmes of care. · Deliver a range of non-pharmacological interventions following specific occupational therapy assessments to enable understanding of any expression of an unmet need and to maximise an individual’s abilities. · Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence. · Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions. · Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process. Detailed job description and main responsibilities The post holder is responsible for the occupational therapy assessment of care needs and contributing to the formulation of the person’s needs and the development of a relevant plan based upon the person’s negotiated goals and relevant evidence based interventions. The post holder, in conjunction with the multi-disciplinary team, will provide occupational therapy interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway.
