We are looking for an experienced Practice Nurse to join our friendly and forward-thinking nursing team.
You will have the opportunity to attend practice meetings, be involved in regular nurse team meetings, participate in clinical supervision, attend PLIs, plus training and education where needed and lead in an area of interest to you.
Main duties of the job
You will need to have chronic disease management experience, Cervical Cytology Training and a strategic understanding of general practice.
About us
Dykes Hall Medical Centre is a 2 site practice based in Hillsborough S6 and Stannington. We currently have approximately 9,000 patients over the two sites. We have 4 partners and 3 Salaried GPs, a well-established nursing team which consists of ANPs, GPNs, and HCAs, and a pharmacy team.
As a training practice, we have GP registrars, F2 doctors, medical students, and nursing students on a regular basis.
Job responsibilities
Undertake tasks that may include:
1. Immunisations, including childhood immunisations and travel vaccinations
2. Following agreed local and national clinical protocols with referral to lead nurse or GPs as appropriate
3. Chronic disease management including diabetes, asthma, and COPD
4. Cytology and Sexual Health
5. Ongoing training and support of the Healthcare Assistants
6. Ordering of Health Promotion Literature
7. Ordering of stock
8. Ordering of vaccinations to maintain stock levels
9. Participation in administrative systems in the practice
10. Maintain accurate records
11. A duty to advise lead nurse of potential problems or errors within the range of assigned tasks

1. An understanding, acceptance and adherence to the need for strict confidentiality.
2. Ability to use own judgment, resourcefulness, and common sense.
3. A commitment to maintain a high professional standard of nursing care and keep up to date with all aspects of nursing care relevant to the post.
4. A commitment to ensure all Health and Safety requirements and Infection Control measures are met and to report any problems to the practice manager.
5. A commitment to the effective use of practice and NHS resources.
6. An awareness of own limitations and experience.
7. To work only in accordance with the NMC and within the Scope of Professional Practice.
8. To have a written professional development plan and to maintain an up to date portfolio which meets the requirements of registration with the NMC including revalidation.
Personal/Professional Development:
The post-holder will participate in any training programme implemented by the practice as part of this employment, such training to include:
1. Participation in an annual appraisal for maintaining a record of own personal and/or professional development.
2. Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.
Person Specification
Qualifications
* First Level Nurse with current NMC registration
* Foundation courses or appropriate education in chronic disease management for asthma, diabetes, COPD
* Aware of accountability of own role and other roles in a nurse led service
* Knowledge of health promotion strategies
* Awareness of clinical governance issues in primary care
* Knowledge of patient group directions and associated policy
* Chronic disease management, asthma, diabetes, COPD. Travel advice and vaccinations, childhood immunisations, contraception reviews, cervical cytology
* Change-management skills and ability to support patients to change lifestyle
* Communication skills, both written and verbal
* Ability to communicate difficult messages to patients and families
* Willingness to undertake education and training appropriate to the role
* Uses initiative
* Gets on well with people at all levels
