We are looking for an individual to develop and deliver a comprehensive high-quality Cardiac Rehabilitation service for patients with a wide range of cardiac conditions in line with national standards.

You will be an active member of the team and will participate in delivering a Cardiac rehabilitation programme to patients, families and carers that is research and evidence based whilst participating in work related audit/research projects in cardiac rehabilitation.
Working in conjunction with the Cardiac Rehabilitation exercise specialist to jointly deliver education, exercise and relaxation components of cardiac rehabilitation managing both medical and surgical patients.
The service is for patients aged 18 years and above and will be delivered in a variety of locations across the Torbay and South Devon NHS Foundation Trust locality. You will be working and reporting directly to the Matron of Cardiology, supporting the department to prioritise daily workload and work autonomously to prevent a patient waiting list.
Main duties of the job
* Teach new staff and students on the cardiac wards and update existing staff about Cardiac Rehabilitation. Act as a resource person.
* Daily clinic to advise/support and assess patients following cardiac events and encourage ongoing rehabilitation and determine suitable programme of rehabilitation.
* Counsel patients and their relatives on the cardiac wards following a cardiac event and prior to and post cardiac surgery.
* Health promotion and long-term risk reduction planning for cardiac patients.
* Provide a telephone answering service for cardiac patients.
* Receive referrals from consultants and the cardiac wards alongside external centres and other health professionals, make decisions on the appropriate follow up care required, and authorise commencement and discharge of cardiac rehabilitation patients.
* Deliver and support the weekly Cardiac Rehab MS Teams Health Promotion talks.
* Collate information for NACR database for audit purposes.
* Use infoflex system to record activity.
* Ensure service runs to agreed standards.
* The role involves looking after patients post MI, PCI, cardiac surgery, Heart Failure, Arrhythmias, and consultant referrals.
About us
The Cardiology department comprises of x2 cardiac catheter laboratories that provide a 24/7 Primary PCI service to a large catchment area. In addition to this, our cardiology department also provides a large range of essential cardiac services such as complex pacing, echocardiography, and advanced cardiac imaging.
There is a 14 bedded Coronary Care/Chest Pain unit and a further twenty-four bedded general cardiology Dunlop wards.

Person Specification
Qualifications
* Registered Nurse.
* Post graduate specialist professional knowledge acquired through training/education or equivalent experience.
* Relevant cardiac course.
* Counselling certificate.
Knowledge
* Specific Knowledge i.e., Health Promotion and Lifestyle.
* Clinical assessment skills re: suitability for programme.
* Recent experience of acute Cardiology or currently in Cardiac Rehabilitation post.
* Research awareness.
* Previous experience of Audit desirable.
