Job summary You will be the lead clinical pharmacist within one of the Health and Well-being teams for one of the five localities in South Devon. Ensuring the delivery of a high quality clinical pharmacy service related to medicines optimisation across their locality. The role will include supporting intermediate care teams, long term condition clinics, care homes and inpatient beds located within the community hospitals. This is an exciting opportunity to provide maternity leave cover in a well established and innovative medicines optimisation team, working in an intermediate care setting with strong links to both acute and primary care multidisciplinary teams. You will provide expert information about medicines and medicines management to a range of audiences including service users, carers and wider trust staff and will ensure services in their locality are patient centred and are delivered in a manner which meets the needs of patients and other stakeholders. Developing and supporting services in order to meet changing operational or clinical needs. This role is a key link between primary and secondary care. Main duties of the job To be the medicines optimisation lead within one of the five localities. To provide a clinical pharmacy service to the locality Health and Well-being team. This includes intermediate care. Lead and deliver a specialist pharmacy service by providing in-depth, patient-focused clinical medication reviews to patients to ensure safe and effective care. Provide highly specialised pharmaceutical advice in highly complex situations (where advice may be challenged) to health care professionals, care home staff, patients and carers. This will involve visiting patients in a range of settings including care-homes, community hospitals and patients' own homes. Promote the safe, effective and high quality, cost-effective use of medicines To provide a clinical pharmacy service to any community hospital beds within the locality, working closely with the local General Practitioners providing the medical cover for these inpatient beds. Attending ward rounds to focus on poly-pharmacy medicine reviews for patients admitted to community hospital wards.
