Job summary As a Senior Community Programme Manager at NHS Humber and North Yorkshire Integrated Care Board (HNY ICB) working within the ICBs Community Health and Care Collaborative, you will play a pivotal role in transforming community health and care services for our population. Main duties of the job You will be responsible for supporting the Strategic Lead to delivery key priorities of transformation across the ICB. You will lead the transformation of multiple projects within community health and care services, working on delivering new models of care and linking together agencies to integrate pathways and improve patient experiences for our population. There are two posts available, one permanent, one fixed term which will have specific portfolios of work reportable to the Strategic Lead.
