As a band 6 Hospital Palliative Care Team member, you will work as part of the team and be supported by the well-established Hospital Palliative Care Team to offer expert care to people with advanced illness and those close to them. You will provide support to a defined caseload of patients, helping to promote and provide dignity, understanding, and compassion.
Base Location: Poole Hospital - based in Forest Holme

Responsibilities include:
1. Providing specialist Palliative Care advice, intervention, and education to patients, families, and healthcare professionals through multidisciplinary team working in the hospital setting.
2. Offering specialist nursing expertise and facilitating evidence-based, patient-centred care for a defined caseload of patients who have complex needs.
3. Providing information and specialist advice in symptom control and psychological support to patients with cancer and non-malignant life-limiting diseases and their families.
