We are looking for an enthusiastic, forward-thinking individual with the ability to work independently in order to manage the needs of our patients.
This is a great opportunity for a Practice Nurse to join our GP Practice in Cobham and be part of a successful and friendly team.
We are flexible in terms of the number of hours and are looking for someone to cover between 20 to 37.5 hours per week.
Main duties of the job
All applicants must be fully qualified Nurses and registered with the NMC. Experience as a Practice Nurse is desirable but not essential.
Ideally, the successful candidate will be able to undertake the following duties (training will be provided if there are any learning gaps): INR monitoring, cervical screening, spirometry, chronic disease monitoring (including diabetes, COPD & asthma), vaccinations, phlebotomy, NHS Health checks, complex dressings, ear syringing, ECGs, and removal of sutures.

We cater for 14,000 patients and are located in a modern, purpose-built Health Centre with an onsite pharmacy. We are a training practice and often have GP Registrars, student nurses, and other allied health professionals on site working with us.

Job responsibilities
Duties and Responsibilities
Clinical:
1. To manage patients by running specific clinic sessions.
2. Work independently and as part of a team to ensure that all chronic disease patients are monitored in line with current clinical guidelines.
3. Provide the following patient care and services (please note, this list is not exhaustive):
* Contraceptive pill checks & Depo injections
* Chronic disease monitoring
* INR monitoring
* Cervical Screening
* Complex wound care
* ECGs
* Health Checks
1. Break bad news to patients/parents/carers and provide appropriate ongoing support to meet identified needs.
2. Act as chaperone when required.
3. Undertake health promotion to help educate patients.
4. Select, carry out and interpret results of specific tests.
Administration:
1. To maintain accurate patient records and enter onto the computer using agreed Read/Snowmed Codes.
2. Ensure accurate completion of all necessary documentation associated with patient health care.
3. To collect data and participate in regular Practice audit and research projects.
4. To contribute to the preparation of any Practice development plans.
General:
1. To maintain own annual registration with the NMC and adhere to Code of Professional Conduct.
2. To maintain a safe and therapeutic environment for all staff, patients, carers, and co-workers. Ensure all statutory regulations (e.g. Health & Safety, COSHH, etc.) are implemented and adhered to.
3. To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice.
4. To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice.
5. Attend and participate in staff and Practice meetings and in-house clinical training sessions.
6. Any other reasonable duties that may be required from time-to-time.

Experience
* Experience of working in a nursing team in a care setting;
* Interpersonal skills;
* Time Management.
* Experience of working in Primary Care;
* Up-to-date training in chronic diseases (including Diabetes, COPD & Asthma);
* Health promotion;
* Family planning;
* Evidence of recent updating in primary care;
* Working with Emis Web.
Qualifications
* RGN Qualification and current NMC Registration.
* Existing Practice Nurse;
* Accredited training in chronic disease management;
* Accredited training in family planning services;
