Principal Responsibilities:
1. Conduct holistic assessments of people with mental health issues and their carers, ensuring support is negotiated, monitored, and reviewed within the first six weeks.
2. Maintain an allocated caseload, managing each case with care and precision.
3. Assess risks to individuals with mental health issues, promoting their independence while ensuring necessary safeguards.
4. Make informed recommendations about the allocation of resources.
5. Promote equal opportunity, respect for diversity, and social inclusion for isolated and vulnerable individuals.
6. Investigate cases of abuse involving vulnerable adults.
7. Collaborate effectively with other agencies to provide comprehensive support.
8. Accurately record decisions and actions, ensuring compliance with policies and legislation.
9. Safeguard the welfare of vulnerable individuals at immediate risk of harm.
10. Engage directly with individuals and carers to conduct thorough assessments.
11. Enhance problem-solving capacities of service users, supporting their independence and choice.
12. Assist service users and carers in making informed decisions to meet their assessed needs.
13. Undertake the role of an Approved Mental Health Professional (AMHP).
14. Perform Mental Capacity Act assessments and make Best Interests decisions, involving advocates when appropriate.
15. Adopt a creative and holistic approach to exploring options for service users.
16. Maximize financial and material resources available to service users.
17. Record unmet needs accurately.
18. Complete assessment documentation according to statutory guidance, legislation, and departmental procedures.
19. Prepare court and specialist reports in the required format.
20. Ensure all case file recordings and relevant documentation comply with departmental policy and procedures.
21. Manage conflicts effectively within individual, family, and professional situations.
22. Justify decisions and uphold professional social work practice with professional assertiveness.
23. Challenge judgements and decisions of others when the needs of service users are not being met.
24. Convene and attend interagency meetings and liaise with relevant parties.
25. Obtain managerial/panel agreement for personal care delivery plans.
26. Record and disseminate care and treatment plans according to statutory requirements and departmental procedures.
27. Shape services directly and indirectly, beyond individual caseloads.
28. Understand budget and resource implications to construct high-quality, value-for-money care provision.
29. Encourage positive risk-taking among service users to promote progress in their lives.
30. Delegate monitoring tasks, if appropriate, following the six-week review.
31. Arrange and coordinate reviews of Care and Treatment Plans for ongoing cases.
32. Engage directly with service users and their carers in the review process, making necessary adjustments.
33. Negotiate and agree on changes to care plans with providers and other involved agencies.
34. Ensure service users are aware of complaints procedures and advocacy services.
35. Mentor newly qualified practitioners and contribute to their professional development.
36. Manage timely administration of all aspects of the caseload.
37. Prepare for and engage in professional supervision with your Line Manager.
38. Establish and maintain positive working relationships with colleagues.
39. Identify and pursue areas for professional development and training.
40. Comply with Welsh language requirements as applicable.
41. Adhere to health and safety advice and guidance.
42. Challenge discrimination and abide by the principles of the Council’s Equal Opportunities Policy.
Safeguarding:
43. This position requires an Enhanced DBS and ISA registration as it is classed as Regulated Activity under the Vetting and Barring Guidance, 2009.
44. This post is exempt from the Rehabilitation of Offenders Act.
Continual Professional Development:
45. Meet the requirements of Social Care Wales for continued registration as a Social Worker.
46. Share skills, knowledge, and learning with colleagues in other specialisms.

48. Undertake post-qualifying training, including practice assessor modules, best interest assessor, AMHP, or post-qualifying assessments.
